1. Complete the Grade 5 Module Reflection Survey

Important: After submission, you will receive a completion code. Record this code carefully. It will be required to verify your participation and distribute PDPs.

2. Complete Applicable Grade-Level Module(s)

Return to the Home page to select and complete any additional Grade-Level modules that apply to your instructional context.
Complete the Reflection Survey for each grade-level module you complete.

3. Complete the Course Evaluation

After completing the applicable modules, return to the Home page to access the Course Evaluation.
Submit the Course evaluation form.
- The Course Evaluation form must be completed to receive Professional Development Points (PDP).
Following submission, MSDE will:
- Review your module reflection surveys and course evaluation for completion.
- Send PDP forms to participants who have met all requirements. Please allow up to 30 days for MSDE to review and process your completion and send PDP documentation.
